Workplace Health, Safety and Compensation Commission has revised its most commonly-used forms. The employer’s report of injury – form 7 – and the worker’s report of injury – form 6 – are two of the forms that have been revised as part of the design and development of a new system to register workplace injury claims.
All workplace injuries, including recurrences of previous workplace injuries, will be registered using the revised form 6 and form 7.
Employers and workers should now be using these new forms. Please discard old versions of the forms at your worksites.
